    O Mundo da Urticária, Com e Sem Alergia

    A urticária é uma entidade clínica comum a um grupo heterogéneo de doenças, devendo ser entendida como um sintoma e não como uma doença. As lesões cutâneas características são pápulas eritematosas ou com zona central pálida e eritema circundante, pruriginosas, que desaparecem à digitopressão e regridem completamente em menos de 24 horas. Embora na maioria dos casos se apresente de forma isolada, pode acompanhar-se de angioedema. A urticária pode atingir até 25% da população em qualquer momento da vida. A forma aguda (duração inferior a 6 semanas), mais frequente em crianças, é mais prevalente que a forma crónica. Nos casos em que a etiologia é identificada, as infecções, os alimentos e os fármacos são as causas mais frequentes. As picadas de insectos e as doenças sistémicas mais raramente estão implicadas. Na urticária crónica é muito raro o envolvimento de fenómenos de alergia IgE mediada. O recurso a exames auxiliares de diagnóstico deve ser criterioso e sustentado por dados clínicos relevantes, não sendo recomendável a requisição de avaliações analíticas exaustivas. Na sua maioria, os episódios de urticária são de curta duração e resolução espontânea. A urticária aguda tem uma duração média de 7 dias; já a forma crónica tem uma evolução muito variável. No que respeita à terapêutica, a eliminação do agente causal é a abordagem ideal, embora nem sempre possível. O tratamento sintomático de eleição são os antihistamínicos H1 não sedativos, em dosagem superior à habitualmente recomendada. Terapêuticas alternativas podem associar-se a estes fármacos, embora não existam fortes evidências científicas que comprovem a sua eficácia. Em algumas situações é importante referenciar a consulta especializada (Dermatologia ou Imunoalergologia); são exemplo as situações IgE mediadas (alergia alimentar ou medicamentosa), ausência de resposta à terapêutica optimizada ou alterações sugestivas de doença sistémica

    Towards zero solid waste: utilising tannery waste as a protein source for poultry feed

    Zero waste is now a strongly emerging issue for sustainable industrial development where minimisation and utilisation of waste are a priority in the leather industry. In a tannery hides and skins converted in to leather through various processes. Approximately 20% (w/w) of the chrome containing tannery solid waste (TSW) is generated from one tonne of raw hides and skins. However, tannery solid waste may also be a resource if it is managed expertly as we move towards zero waste. This research illustrates the potential of tannery solid waste as a poultry feed additive. An oxidation method was used to achieve 95% of dechroming rate of chrome tanned waste followed with thermal and enzymatic treatment to produce gelatin solution and collagen concentrates. The thermal stability and fibre structure of samples were analysed by Differential Scanning Calorimeter (DSC) and Scanning Electron Microscope (SEM). Protein content and fourteen amino acid concentrations were determined using amino acid analysis. High Performance Liquid Chromatography (HPLC) was used to compare the amino acid composition with wheat and soya bean meal that is conventionally used in poultry feed. The nutrient requirements for poultry feed vary according to the purpose for which they have been developed. The high content of arginine, leucine, threonine, serine and methionine in the extract were of a sufficient level for poultry feed. Hexavalent chromium test was performed and showed that levels of the metal were low enough to be used in feed additives. In addition, the extracted product showed 75% digestibility (in vitro) and appears that treated TSW may be utilised in poultry feed, this demonstrates a clear example of waste utilisation. In Bangladesh plans are being formed to use the extract in poultry feed production

    Quantification of Plasma and Urine Thymidine and 2'-Deoxyuridine by LC-MS/MS for the Pharmacodynamic Evaluation of Erythrocyte Encapsulated Thymidine Phosphorylase in Patients with Mitochondrial Neurogastrointestinal Encephalomyopathy.

    Mitochondrial neurogastrointestinal encephalomyopathy (MNGIE) is an ultra-rare disorder caused by mutations in TYMP, leading to a deficiency in thymidine phosphorylase and a subsequent systemic accumulation of thymidine and 2'-deoxyuridine. Erythrocyte-encapsulated thymidine phosphorylase (EE-TP) is under clinical development as an enzyme replacement therapy for MNGIE. Bioanalytical methods were developed according to regulatory guidelines for the quantification of thymidine and 2'-deoxyuridine in plasma and urine using liquid chromatography-tandem mass spectrometry (LC-MS/MS) for supporting the pharmacodynamic evaluation of EE-TP. Samples were deproteinized with 5% perchloric acid (v/v) and the supernatants analyzed using a Hypercarb column (30 × 2.1 mm, 3 µm), with mobile phases of 0.1% formic acid in methanol and 0.1% formic acid in deionized water. Detection was conducted using an ion-spray interface running in positive mode. Isotopically labelled thymidine and 2'-deoxyuridine were used as internal standards. Calibration curves for both metabolites showed linearity (r > 0.99) in the concentration ranges of 10-10,000 ng/mL for plasma, and 1-50 µg/mL for urine, with method analytical performances within the acceptable criteria for quality control samples. The plasma method was successfully applied to the diagnosis of two patients with MNGIE and the quantification of plasma metabolites in three patients treated with EE-TP
